Veterans Affairs Assessments (Medical Questionnaires)

We do not offer general psychological assessments or psycho-educational assessments.

We do perform comprehensive assessments with Veterans, Members of the CAF, and RCMP for the purposes of completing the Veterans Affairs Medical Questionnaire for a mental health condition. These are assessments required by Veterans Affairs Canada to apply for and be re-assessed for mental health disability awards (previously known as pension awards). If you think your service has caused or exacerbated a mental health condition, or VAC has recommended you apply for a mental health disability award, please contact us via the “Request an Appointment” button below to schedule an assessment.

Sessions are offered either virtually (phone or video) for individuals anywhere in Ontario or in-person in London, Ontario.
